Community Well-Being represents our commitment to improving health outcomes in our community through rigorous assessment of health status in our region, incorporation of stakeholder’s perspectives, and adoption of related implementation strategies to address priority health needs. The goals of this hub are to provide data to support this commitment with the following tools:
- Data indicators for assessing and understanding the community’s health behaviors, risk factors, and social determinants that impact health. These data indicators are calculated at the primary and secondary service areas.
- Make a Map to provide a customizable visual representation of selected indicators.
